How to Fill Out a Health Card Renewal Form?

Before completing the Ontario Health Card Renewal Form you will want to make sure you are completing a separate form for each person who will need to renew their health card. Once the form is completed you will need to print it out and take it in person to your local ServiceOntario center along with one original document from both Lists 2 and 3 of the Ontario Health Coverage Document List as proof of citizenship or legal immigration status.

You will need to provide the following information on Form 4297-82:

1. Current Health Number and Audit ID.
2. Your full name and mailing address. Note if your mailing address differs from your residential address, you will need to complete both sections. There is also a section where you can update your mailing or residential addresses.
3. Date of birth.
4. Your language preference.
5. Sex.
6. Above the signature, the confirmation box will be a series of statements you agree to be accurate and up to date to the best of your knowledge. This section also states that if there are any changes you need to make to your name, address, immigration, or citizenship status after completing this form, that you will inform the Ministry of Health within days. If it is found that your form contains inaccurate information you run the risk that your form will be rejected, which could ultimately put your health card renewal in jeopardy.
7. Name, signature, home, and work telephone numbers, and date sections. There will also be a checkbox to clarify who completed the form (whether it is the applicant themselves, a parent or guardian, or power of attorney).
