Form T4E Statement of Employment Insurance and Other Benefits - Canada (English / French)

EI Tax Form Instructions

Form T4E consists of two identical timetables. One of them is supposed to be filed according to the taxpayer's income tax return, the other should be kept in the taxpayer's records. The tax document requires the following information:

1. Taxpayer's Identifying Information. Even though the document is supposed to be attached to the federal return, the taxpayer still has to identify themselves. For this purpose, they must indicate their social insurance number.
2. Benefits Information. In this part of the form, the taxpayer must fill in different amounts, such as repayment rate, deducted income tax, the total amount of the benefits they received, etc. In addition to this, the taxpayer must indicate the year when they have received these amounts.
3. Repayment Chart. Here the taxpayer must state certain amounts from the previous part of the document. They are required to do so only if their circumstances match the situation described at the beginning of this chart.
4. Instructions. Form T4E provides taxpayers with guidelines on how to fill out each gap in the form. If the taxpayer still has questions related to the amounts they are required to designate, they can visit the official website of the Government of Canada for more information.
