A Speech Evaluation Form is a written survey completed to rate the speaker and provide feedback on the speech. Whether you organize a workshop with multiple speakers and topics of discussion or you work for an educational institution that examines the students' ability to give a good speech, this document can come in handy - print it out and distribute it among the people who have attended the event, lecture, or seminar or send the form to the participants via e-mail.
