NOTICE OF INTENTION TO EXHUME BODY OR
RECOVER CREMATED REMAINS
To:
(insert name of person in charge of the place where the body is or the cremated remains are)
and to:
(insert name of person State Coroner considers has a sufficient interest in the autopsy)

give notice that I intend to make the following order:
that the body of:

buried at:
(Insert details of place where body is buried, eg, name and location of a particular cemetery)
be exhumed to enable an autopsy of the body to be conducted;
or
that the cremated remains of

(Insert details of place where body is buried, eg, name and location of a particular cemetery)
be recovered to enable an autopsy of the cremated remains to be conducted.

Note: Section 20(3) of the Coroners Act 2003 provides that at least 2 days notice must be given except in certain
circumstances.
