The purpose of this form is to dispute a vehicle’s fuel type for registration purposes.

Effective January 1, 2020, Ohio Revised Code (R.C.) section 4503.10(C) requires an additional $100/Hybrid or $200/Electric fee
per registration year. The Ohio Bureau of Motor Vehicles (BMV) utilizes a VIN Decoding Program provided by the National
Highway Traffic Safety Administration (NHTSA) to determine a vehicle’s fuel type and properly calculate the registration fees.
In order to dispute your vehicle’s fuel type after paying your registration fee, please complete this form and mail to the
address below with your supporting documentation. This form cannot be processed by the license agency and must be
mailed directly to the BMV.

If the BMV determines that the fuel type reported from NHTSA is incorrect, the BMV will issue a refund of the Hybrid or Electric
fee collected at the time of the registration transaction, as appropriate, to the vehicle owner listed above.
I have been advised that my vehicle’s fuel type was determined by NHTSA, but I am disputing what NHTSA is
reporting. I affirm that the fuel type for the above described vehicle is the following:
GASOLINE / DIESEL / OTHER FUEL TYPE
HYBRID – A passenger car powered by an internal propulsion system consisting of both of the following:
1) A combustion engine;
2) A battery cell energy system that cannot be recharged via an external source of electricity but can be
recharged by other vehicle mechanisms that capture and store electric energy.
ELECTRIC – A passenger car powered wholly or in part by a battery cell energy system that can be recharged
via an external source of electricity.
NOTICE: Falsifying this application is punishable by fine and imprisonment (R.C. 2913.42).
