The purpose of this form is to declare a vehicle’s fuel type for registration purposes.

Effective January 1, 2020, Ohio Revised Code (R.C.) section 4503.10(C) requires an additional $100/Hybrid or $200/Electric fee
per registration year. The Ohio Bureau of Motor Vehicles (BMV) utilizes a VIN Decoding Program provided by the National
Highway Traffic Safety Administration (NHTSA) to determine a vehicle’s fuel type and properly calculate the registration fees.
To process your vehicle registration, please select the appropriate option below and identify the fuel type:
I have been advised that NHTSA was not able to determine my vehicle’s fuel type. I acknowledge that my vehicle’s
fuel type may be updated by NHTSA in the future, which may result in an obligation to pay the $100/Hybrid or
$200/Electric fee, as appropriate. Therefore, I affirm that the fuel type for the above described vehicle is the following:
GASOLINE / DIESEL / OTHER FUEL TYPE
HYBRID – A passenger car powered by an internal propulsion system consisting of both of the following:
1) A combustion engine;
2) A battery cell energy system that cannot be recharged via an external source of electricity but can be
recharged by other vehicle mechanisms that capture and store electric energy.
ELECTRIC – A passenger car powered wholly or in part by a battery cell energy system that can be recharged
via an external source of electricity.
NOTICE: Falsifying this application is punishable by fine and imprisonment (R.C. 2913.42).
