Obtain/request an SP-101 number by emailing fleetcoordinatorsgroup@azdot.gov.
1.
Enter the number and/or date on Vehicle Disposal Form SP-101 (top right corner) on all pages with vehicles.
2.
Complete the rest of the SP-101 Vehicle Disposal Request Form and Save.
3.
Send completed form to your Asset Manager for approval signature (Required). (See signature instruction below.)
4.
Send/forward approved form to fleetcoordinatorsgroup@azdot.gov.
5.
The requester will be contacted by an ADOT Fleet coordinator when vehicles have been approved for delivery.
