What Is a BOTOX Consent Form?

A Botox Consent Form is an informed consent document every patient that wants to get a Botox injection for medical or cosmetic reasons needs to sign to confirm they are aware of the possible side effects of the procedure. Whether the patient wishes to reduce the skin wrinkles or they are seeking treatment for various muscle spasms, they have to know about the complications they may deal with during or after the injection.
