Filing Instructions
A.
Application must be typewritten or neatly printed.
B.
Submit a check or money order in the amount of $75.00 Payable to: Treasurer, State of Ohio.
C.
This permit is for explosive manufacturing/processing. This permit is required for the manufacturing/processing of Ammonium
Nitrate (AN) in excess of 5,000 pounds.
D.
Submit a copy of the required corporate surety bond in the principal sum of $1,000,000 or a public liability insurance policy for the same
amount, for the purpose of the payment of all damages to persons or property which arise from, or caused by, the conduct of any
act authorized by the permit upon which any judicial judgment results.
E.
Submit an FBI background check. The State Fire Marshal may accept a current valid "Certificate of Clearance" issued by the Bureau of
Alcohol, Tobacco, Firearms and Explosives (ATF), Transportation Security Administration (TSA) Hazardous Materials endorsement, or TSA
Transportation Worker Identification Credential (TWIC) card as satisfactory documentation of a person's criminal history if issued
within 36 months of your application.
F.

SUPPLEMENTAL PERMIT FILING INSTRUCTIONS FOR EXPLOSIVE
MANUFACTURING/PROCESSING
The Ohio Administrative Code (OAC) 1301:7-7-56(A)(2)(f) requires that permit applicants must complete an FBI
background check. To better serve our customers, our Bureau is now using the Webcheck Electronic Criminal Background
Check system. This system uses fingerprints as well as an individual's social security number and date of birth. This
service is provided in our office for $25.25. A listing of other agencies that have the Webcheck system for those that are
unable to come to our location can be viewed at

The State Fire Marshal may accept: an Ohio conceal & carry permit or a current; a current, valid “Certificate of Clearance”
issued by Bureau of Alcohol, Tobacco, Firearms and Explosives (ATF); Transportation Security Administration (TSA)
Hazardous Materials endorsement; or TSA Transportation Worker Identification Credential (TWIC) card as satisfactory
documentation of a person's criminal history if these were issued within thirty-six months of your application.
The following information should be provided when applying for an explosives storage and/or explosive material
storage permit:
1.
A map indicating the location of the explosives storage and explosive material storage, and if applicable,
magazine(s) and directions for the inspector. (A county highway map is acceptable)
2.
A complete address (Please do not use P.O. Boxes)
3.
List the location of all explosives storage and explosive material storage and/or total number of magazines at
each location or site. (only one permit for each location or site of the magazine(s).
4.
List the contents and class of each magazine, the weight of the explosives in pounds or the number of blasting
agents in each magazine.
5.
Indicate the distance of each explosives storage and explosive material storage location and/or each magazine
from the nearest building(s), highways, railway(s), and from each magazine.
6.
A copy of the corporate surety bond in the principal sum of $100,000 or a public liability insurance policy for the
same amount, for the purpose of the payment of all damages to persons or property which arise from, or caused
by, the conduct of any act authorized by the permit upon which any judicial judgment results.
7.
Provide phone number(s) of the responsible person(s) who will accompany our inspector to the site or location
for the purpose of inspection.
The regulations governing permits for explosives storage and explosive material storage are in rule 56 of the Ohio Fire Code.
NFPA 495 2013 edition is the standard for explosive storage and use, incorporated into the Ohio Fire Code by reference.
When the above information and completed application is received by our office, a fire safety inspector will visit the site to
conduct an inspection. Upon receipt of the inspection report, your permit will be issued if no code violations are found.
Please check with any other agency (i.e. the local fire code official) to ensure that you are in compliance with all other
requirements for permitting/licensing at the local and federal levels.
NOTE: When a permit is not required by the local fire code official to engage in any hazardous activity set forth in the
appendix to rule 1301:7-7-01 of the Administrative Code (Ohio Fire Code), the person intending to engage in the hazardous
activity shall first notify the local code official of that person's intent to engage in that hazardous activity.
