Prior to the issuance of a Certificate of Occupancy/Completion for the above referenced building or part

thereof, the undersigned contractor by signing below does hereby certify that he/she personally supervised the

construction of the above mentioned building or part thereof and that said building or part thereof has been

constructed in all respects in compliance with the Building Code of the City of Orlando, the same being

Chapter 13 of the Code of the City of Orlando, as amended, all other City ordinances and regulations relating

to the above mentioned project and in accordance with the plans and specifications as approved by the City of

Orlando and does hereby assume complete responsibility for the construction thereof.

engineering and building inspections as required, including final inspections prior to the issuance of the

Certificate of Completion

A certificate of completion will be issued, upon request, for completed alterations to structures already
