What Is a Student Information Sheet?

A Student Information Sheet is a mini portfolio that contains information about a student. They are mainly used by teaching staff and are a great way to build your relationship with students. It shows them that the teacher in front of them genuinely cares and wants to listen, taking an interest in their lives, hobbies, and plans for the future. They can be filled in by either the student or their parents.

The provided information can be used by the teacher to give advice and provide support to students if this is required. It also helps any member of the teaching staff to get to know their students better and where appropriate, use this information in an educational setting to motivate students.

Naturally, it is best to hand out such sheets at the start of the academic year particularly if you have a new class with unfamiliar faces. Even if you know the students in your class, students grow up quickly and their interests may change even quicker so it may be worth handing this sheet out regardless of their previous history.

What to Include on a Student Information Sheet?

You should think carefully about what should be included on a Student Information Sheet to make sure that you collect all of the relevant information about each student. Below are some examples of things that would be worth including in a standard Student Information Sheet:

  • Start by collecting the basic details of the student including their full name and date of birth;
  • Request the contact information along with the names of a parent or legal guardian that the teacher and/or school can get in touch with in case of an emergency or another serious matter;
  • You can ask how the child gets to and from school. This is particularly useful for younger students that are allowed to walk home alone. Teachers need confirmation from the parent that their child can do this independently;
  • Leave room for the parents to detail any medical conditions that their child could have and maybe even basic information on what should be done if a condition is triggered;
  • You can also leave room for students or their parents to describe any hobbies or interests that a student may have;
  • If you are working with older students, you can also leave room for them to note down their plans for the future, what they would like to do, they can then describe any personal achievements that they have made and talk about what makes them unique;
  • You can also leave room for students to write down what their expectations are from the educational program or course - what they hope to take from it and what they want to achieve.

Compiling this information will give teachers a clearer view and a full profile of the students that they have within the classroom.
