Form HSMV82101 Application for Duplicate or Lost in Transit / Reassignment for a Motor Vehicle, Mobile Home or Vessel Title Certificate - Florida

Form HSMV 82101 Instructions

Provide the following information in the HSMV 82101 Form:

1. Select the type of application. You might need a new title if the other was lost or stolen, lost in transit, or you wish to obtain a duplicate for a transfer. Both a seller and buyer must sign the form for the latter type.
2. State the personal details of the owner - the full name, mailing address, and email address. If applicable, you may add the same information about the co-owner, purchaser, and co-purchaser. Additionally, the purchaser's date of birth and driver's license or identification card number are required.
3. Indicate who makes the application - the owner, lienholder, or dealer. The lienholder must record the date of lien; the dealer must record the dealer's license number.
4. Describe the vehicle, mobile home, or vessel. Write down its identification number, make or manufacturer, year, body, color, license plate or registration number, and the Florida title number.
5. State the future usage of the vehicle - a lease, for police needs, as a taxi, private use, etc.
6. Provide the lienholder information - name, date of lien, contact details, a Federal Employer Identification Number, driver's license number, sex, and date of birth. The lienholder may authorize the Department to send the title to the owner.
7. Record the current odometer reading. It is possible to indicate the odometer discrepancy or indicate the odometer has exceeded its mechanical limits.
8. Certify that you have read and understood the form. If the item is transferred, describe how the transferee acquired it and add the date and price of the transaction.
9. Sign the form. If there are any purchasers, co-sellers, co-purchasers, or lienholders, each party must provide a signature once the form is completed.

Form HSMV 82101 fee is required only if you need a duplicate title. The fees depend on the type of transaction:
