Instructions for Form 4230, Affidavit Filed by an Innovations
Center for Claiming Property Tax Exemption
As a supplement to the following instructions, please read State Tax Commission (STC) Bulletin 11 of 2004 which explains
the provisions of Public Acts 244 and 245 of 2004. This bulletin can be found on the Michigan Department of Treasury
Web site at www.michigan.gov/treasury. When you reach the site, click on Local Government and then click State Tax
Commission.
LINE-BY-LINE INSTRUCTIONS:
Line 1: P.A. 244 and 245 of 2004 state that the applicant must be an Innovations Center. See the STC Bulletin 11 of 2004
for a definition of an Innovations Center.
Lines 2-4: Indicate the county, the city or township or village, and the local school district in which the Innovations Center is
located.
Line 5: The exemption can include real and/or personal property. If you are claiming the exemption for real property, attach
the legal description of the property. If you checked that the claim is for all personal property of a Qualified
High-Technology Business, attach a list of the name(s) and address(es) of the Qualified High-Technology Business(es).
Line 6: If there is someone in your business, other than the person signing this application, who should be contacted if
further information is needed, please name the person in line 6.
Line 9b: Note that a signature is required on line 9b.
