Form REG 256, Statement of Fact , is a document released for those individuals who own a vehicle or vessel and want to make various official statements regarding them. These statements may include a use tax exemption, smog exemption, a statement for a transfer or a title, vehicle body change, etc.

How to Fill Out a Statement of Facts Form?

The Statement of Facts Form is a complex document that consists of several sections. The types that a filer should complete depends on the subject of their application. These sections include:

  • Statement of use tax exemption . Here a filer must designate the reasons why this transfer is exempt from the use tax, such as a transfer between family members, gift, court order, inheritance, etc.;
  • Statement for smog exemption . In this section, an applicant must state why their vehicle doesn't require smog certification. For example, a vehicle is electric or diesel-powered;
  • Statement for transfer only or title only . If a car has not been used or parked on a street, its owner can apply for a transfer only or a title only;
  • Window decal for wheelchair carrier or wheelchair lift . Individuals who have a Disabled Person License Plate, or a Disabled Veteran License Plate, or a Permanent Disabled Person Parking Placard, can use this section to apply for a window decal for their vehicle;
  • Vehicle body change statement . When a filer has changed their vehicle's unladen weight, motive power, body type, or a number of axles, they must report it by completing this section. An ownership certificate is required;
  • Name statement . This section is supposed to be filled in if a filer is applying for a correction of a misspelling mistake in their name, or for a name change. In this case, an ownership certificate is required as well;
  • Statement of facts . Applicants can use this section to make a statement that everything mentioned in this document is true and correct.

According to the REG 256 Instructions, an applicant must fill out the first section dedicated to the vehicle description, and the appropriate section mentioned above. In the first part of the form, they must designate the license plate number (or a CF number), vehicle (or vessel) ID number, its year and make. Regardless of the subject of the application, a filer must always fill in the last section - The applicant's signature. Here they must designate their full name, date, and a daytime telephone number where they can be reached.

After an applicant has completed the form, they can submit it online on the official website of the California DMV or file it personally in their local DMV office.
