What Is Optional Form 306?

OPM Optional Form 306, Declaration for Federal Employment , is currently used to assess a person's eligibility for Federal contract employment as well as enrollment in the Government's Life Insurance program. You may be asked to complete this form at any point during the hiring process. This form is also used to assess an applicant's security clearance suitability and may be disclosed in the future to other authorized officials.

Declaration for Federal Employment Instructions

The Declaration for Federal Employment Form is divided into seven sections, many with multiple questions that must be answered. Any false statement made on this form can result in you not being hired for the position, firing if employed, a fine, or imprisonment.

  1. The first section asks for the general information of the applicant, including any other names used before.
  2. The second section asks if the applicant is part of the Selective Service Registration and if the applicant has ever been registered in the Selective Service System.
  3. The third section asks for the applicant's military service history, including the branch and type of discharge.
  4. The fourth section asks several background questions. The first questions of this section will ask about prior convictions or fines greater than $300, including any military convictions. The next question of this section asks if you have been fired, left by mutual agreement, or quit upon learning you would be fired from a position in the last 5 years for any reason. The last question asks about any delinquencies of Federal debt.
  5. The fifth section asks if you have any relatives working in the same government organization that you are applying to and if you are currently receiving any pension or retirement from a Federal, military, or District of Columbia Government agency.
  6. The sixth section is an additional comments section for responses to previous questions where they ask you to elaborate on your answers.
  7. The seventh section is a signature section for applicants who have not yet been selected for a federal position and also has a final set of questions for appointees. These last questions ask if you have been enrolled in or waived previous life insurance for past government positions you have been appointed to.
