What Is a DMV REG 124 Form?

Form REG 124, Application for Assigned Vehicle Identification Number Plate, is a legal document completed to receive a new California vehicle identification number (VIN) for your motor vehicle. It is one of the documents you need to fill out when registering your vehicle in the State of California. Visit your local Department of Motor Vehicles (DMV) office to request a packet of forms to register your car.

Since October 2012, DMV REG 124 Form is not available on the website and the California DMV will not let you download a blank template. However, they will present you with a completed version to provide authorization to get an appointment with the California Highway Patrol (CHP).

The latest version of the form was issued by the California DMV in June 2010 with all previous editions obsolete. Form 124 REG must be signed by the DMV representative so that later the CHP identifies your vehicle, verifies the information in the application, and assigns a VIN on the cowl or doorpost of your car.

California DMV VIN Registration

When the physical inspection of your vehicle is completed, the CHP will issue Form CHP 97C, Certificate of Inspection, and you will have to submit it with the vehicle registration application. As an alternative, a CHP employee will complete Form REG 31, Verification of Vehicle, and you will sign it.

One of the common reasons for completing the REG 124 Form is the CHP salvage vehicle inspection. Your salvage vehicle might be referred to the CHP if it was in an accident and the driver’s door was changed, the airbags were not repaired properly, the VIN tag is altered, mutilated, missing, or is different from the VIN on the supporting documents, or you became a part of the random enforcement program which referred you to the CHP.

To fill out this application, you need to describe your vehicle – write down its California license plate number, make, year, model, identification number and engine number (if they are mentioned in any of the documents). Explain the reason for your request or referral. State your full name and current address. Certify the information in the form is true and correct, you are the lawful owner of the vehicle, sign, and date the application.
