What Is Form VTR-34?

Form VTR-34, Application for a Certified Copy of Title , is a form that should be completed by the owner of a vehicle to replace a Texas Certificate of Title in case the original document is lost, destroyed, or mutilated. If there is a lien on a vehicle, a VTR-34 Form must be filled in by the lienholder. If an applicant is not a lienholder, they should provide an original release of lien. A Certified Copy of Texas Title can be requested to replace the original certificate or any previously issued copy of it. An applicant who needs to apply for a Certified Copy of Title should wait 30 days after the last title was issued.

If the owner or lienholder of a vehicle needs to obtain the information contained in the motor vehicle record, Form VTR-34 is not suitable for this purpose. They should complete Form VTR-275, Request for Texas Motor Vehicle Information in this case. A record request can be submitted to obtain a title history, or current or expired title and registration verification of a vehicle record.

VTR-34 Instructions

VTR-34 Form instructions are the following:

  • Information . This section contains reference information about the form;
  • Application Checklist . This section includes a list of the requirements connected with completing and submitting the form. An applicant has to check each box to ensure their completion;
  • Vehicle Information . The filer should enter information about the vehicle, such as vehicle identification number, year, model, title/ document number, and Texas license plate number;
  • Recipient Information . Provide information about the recipient of the Certified Copy of Texas Title, such as their full name, mailing address, email, and phone number;
  • Government Issued Identification . An applicant has to indicate the type of their identity document, the state or country of issue of this document, its ID number, and expiration date;
  • Certification . The filer has to sign the form, print their name, and indicate the date of completion. It is allowed to sign with the use of black or blue ink only;
  • Applicant Identification Requirements . This section contains detailed rules of confirming government-issued identification depending on the type of the applicant. The filer can find there as well a list of acceptable forms of photo identification, the rules of submitting connected with the lien on a title record, conditions and cost of applying.

The owner or lienholder should attach to the completed VTR-34 Form a copy of their government-issued photo ID. If there are several owners of a vehicle, each owner should sign the Application for a Certified Copy of Title and provide a copy of their government-issued photo ID. The filer has to attach to the form a document confirming payment of the applicable fee as well. Its amount depends on the method of applying. These fees are non-refundable.

An applicant should submit the required documents by mail or in-person to the Texas Department of Motor Vehicles (TxDMV).
