What Is Form VTR-275?

Form VTR-275, Request for Texas Motor Vehicle Information, is a form that should be completed by the owner or lienholder of a vehicle to request information contained in the motor vehicle record. The reasons for submitting a record request can be the following:

  • Obtaining a Title History;
  • Certified Title History;
  • Current or expired Title and Registration verification of a vehicle record;
  • Duplicate registration receipt for the current registration period.

Texas motor vehicle information can also be requested with the use of this form by a government entity in performing its functions and for producing statistical reports.

If the owner or lienholder of a vehicle needs to replace the Texas Certificate of Title because the original certificate is lost, destroyed, or mutilated, Form VTR-275 is not suitable for this purpose. They have to complete Form VTR-34, Application for a Certified Copy of Title in this case.

VTR-275 Form Instructions

A VTR-275 Form must be filled in as follows:

  • Information and Instructions. This section contains reference information about the form;
  • Applicant Information. The filer has to provide their personal information, such as their full name, address, email, and phone number;
  • Government Issued Photo Identification. An applicant has to indicate the entity issuing their ID and its type, enter their government photo ID number;
  • Search Information. The filer should provide information about the vehicle, such as its Texas license plate, vehicle identification number, year, and title document number;
  • Records Request. This section contains a list of reasons for submitting the form, and the filer has to choose one of them. This section involves as well, the amount of the fee that should be paid depending on the type of request. These fees are not refundable;
  • Certification. The filer has to enter their name and date of completion. Signing this form, an applicant certifies that the information provided is correct.
  • Permitted Use. An applicant should indicate the status that authorizes them to obtain vehicle information. The filer has to check the appropriate box depending on whether it is the current owner/ lienholder of the vehicle or an authorized representative of the recorded owner/ lienholder. If an applicant is a representative of another authorized organization, they should choose a purpose to obtain vehicle information from the list of the permitted use on Page 2 of the form.
  • Resale and Redisclosure. This section explains the rights and obligations of the recipient of vehicle information.

An applicant should attach to the completed Form VTR-275 a copy of their government-issued photo ID and a document confirming payment of the applicable fee. If an applicant is not the current owner or lienholder of a vehicle, they have to attach to the form a written authorization from the recorded owner or lienholder. These documents should be mailed to the Texas Department of Motor Vehicles (TxDMV).
