GENERAL PERMIT FOR BLUESTONE MINING
RENEWAL OF COVERAGE UNDER BMP-GP-105
Instructions
Complete this form only if you have existing coverage under BMP GP-105 and wish to renew the coverage as it currently exists. If
the area is changing, a new NOI will be required and a new authorization number will be assigned.
Supply the basic information on the following form and include attachments as necessary.
SECTION A. APPLICANT INFORMATION: Complete all items in this section, including any updates to the original NOI. Be sure to
include a valid email address that will be regularly checked.
Indicate your previous registration number (GP-105
) and its expiration date. This number will be retained for the renewal.
SECTION B. RENEWAL: By completing this form you are requesting up to 10 years of coverage as granted in the General
Permit BMP-GP-105.
If you are not changing any additional information from the original submittal of your Notice of Intent (NOI) (other than provided in
Section A.), check that the previous NOI information still applies for the additional 10-year period.
Updated information is required for renewals. Provide the following with the Renewal request:
Mining and support acreage progress report and bonding calculations: Provide the number of acres used for mining (not to
exceed 5 acres) and support acreage. Indicate what has been affected/used so far. Calculate using the bond rates in effect for
the current year as published in the Pennsylvania Bulletin.
The Department will request additional bond if necessary.
Verification of right of entry documents for next 10 years: If the original NOI contained this documentation that covered the
additional 10 years requested, no additional document is necessary. If a land lease is expired, a new lease must be provided.
Updated maps and cross section(s): Provide a map showing existing and proposed conditions.
Renewal of NPDES: Stormwater GP-104 (Form 5600-PM-BMP0008) or Individual NPDES (Form 5600-PM-BMP0032) coverage
must remain for the permit area and be renewed every 5 years.
A $550 fee is required for this renewal of coverage, which is the same as the initial permit coverage.
SECTION C. CERTIFICATION: Sign the page and have it notarized. Submit the original to the District Mining Office.
