What Is DA Form 1506?

This is a military form that was released by the U.S. Department of the Army (DA) on January 1, 2022. The form, often mistakenly referred to as the DD Form 1506, is a military form used by and within the U.S. Army. As of today, no separate instructions for the form are provided by the DA.

How to Fill Out DA Form 1506?

The Army Statement of Service must be filled out by the career counselor or the human resources command analyst - the responsible personnel must confirm the duty described in the form after the soldier re-enters active duty after taking a break and show all prior service periods. Only approved documentation can be taken into account - follow these DA Form 1506 instructions to establish creditable service:

  1. Write down the full name of the soldier and the identification number assigned to them by the Department of Defense . It is also necessary to state what unit they belong to and its mailing address.

  2. Describe the periods of service . It is permitted to list up to fourteen periods of service - specify the branch of service for every period, enter the soldier's status, and confirm whether the service period is credit for active duty, pay, or neither. Add the beginning and the ending dates of every service period. State how many active duty points the soldier has obtained and how many days of lost time every service period includes. You have to refer to the source that allowed you to fill out the statement - note that in case a certain document is missing from the soldier's file, the records must be updated. If you run out of space to list all the service periods, you can use a DA Form 1506-1, Statement of Service (Continuation Sheet).

  3. Carry out the service computation - active duty must be defined by years, months, and days . The service for pay and active service are separated into two tables - calculate every period of service and enter the total numbers in the last row of every table. You will also have to calculate the total numbers reflected on the continuation sheet separately from the main document.

  4. Determine the service dates - use the previous computation results and convert them into actual calendar days both for the Pay Entry Base Date and the Basic Active Service Date . Once the calculations are done, certify the instrument by signing it. After the career counselor or human resources command analyst completes the paperwork, their senior or team lead is expected to provide authentication and upload the statement into the Army Military Human Resources Record. In case the statement is used to correct errors, it will be submitted to the personnel office by the career counselor - the adjustments to pay supported by the filed documentation, including the Statement of Service, will take place without delay.
