Presentation Evaluation Form

A Presentation Evaluation Form is a written survey prepared to find out opinions about a presentation - a public seminar, exhibition, lecture, new product launch, etc. The person or business in charge of organizing this event may distribute this short document among the participants after the presentation to collect feedback, learn what they think about the presentation, and discover ways to improve especially if the presentation is supposed to be a recurring event.

A Presentation Evaluation Form template can be downloaded below. It is up to you to add whatever is necessary to the template depending on the presentation; however, at a minimum, this form should name the presenter and include the date and topic of their presentation (which is crucial if the participant has listened to multiple speeches or lectures during a short period of time), indicate the status of the individual providing their point of view, and list various criteria to assess the presentation with scores - for instance, a lecture attendee may be asked whether the presentation was easy to follow and understand and whether the audience seemed interested in the topic.
