What Is a Borough Intake Form?

This Borough Intake Form is a supplementary document you need to file when submitting manual applications to the Borough Office. Its purpose is to improve tracking of all the filed documents and allow the authorities to send you a denial notification electronically, instead of making you go back to the office to take the paperwork. A separate Borough Intake Form is required for each application you submit. It is possible to attach supporting documents - for instance, PW1A Schedule A: Occupancy/Use or PW1B Schedule B: Plumbing/Sprinkler/Standpipe.

How to Fill Out NYC DOB Borough Intake Form?

The Borough Intake Form instructions are as follows:

  1. Indicate the number of the application and the date of filing. Provide the location information, name of the requestor, and the relation to the job (owner, applicant of record, contractor, or filing representative). Write down the email addresses and state whether you are filing an initial application or resubmitting documentation. If it is an initial submission, affix a B-SCAN job sticker to the documents to allow the DOB to scan and electronically store your papers;

  2. Select the type of request. File a Borough Intake Form for each of the following documents:

    • PW1 (Post Approval Amendments). Submit Form PW1, Plan/Work Application, to correct an error in your initial filing. After the DOB approved your application and plans, modifications are very common as the job progresses. Even if you need to change a little detail in the work, somehow alter a schedule or scope of work, you have to file Form PW1 to approve this change. It will be reflected in the NYC Building Information System (BIS) and you can keep your construction project on schedule. To notify the DOB of the change, choose "Post Approval Amendments" in the section "Filing Status" and add details in the section "Comments";
    • PW1 Withdrawal. Form PW1, Plan/Work Application, may be withdrawn using the same form. Whether the applicant needs to withdraw entirely from the job or from a portion of it, this is the way to do it. Select the "Withdrawal" option in the section "Filing Status" and state the reason for withdrawal in the comments. If you need to withdraw from the portion of the job, indicate the existing document number and work types affected by the filing;
    • Form PW2, Initial Work Permit Application. This form is filled out by contractors and licensees authorized by the owner of the premises that allows them to obtain a permit to perform work. It allows the applicant to obtain an initial permit or renew an older one with or without changes. If no physical work will be performed under this permit, select the "No Work Permit" option on the form;
    • Form PW6, Certificate of Occupancy Inspection Application (Initial). Before the DOB issues the Certificate of Occupancy, the inspections must be performed. A registered architect, professional engineer, or construction superintendent completed this form to request the inspection from the DOB;
    • Form PW7, Certificate of Occupancy/Temporary Certificate of Occupancy/Letter of Completion. File this document to obtain a Letter of Completion, Temporary Certificate of Occupancy (TCO), or Final Certificate of Occupancy from the Borough Commissioner's Office. Additionally, you may use this form to renew the TCO. Each Form PW7 must be accompanied by a new B-Scan job sticker.

The bottom of the page contains the reasons for denial that will be indicated by the borough staff and emailed to the addresses you included in your intake form.
