What Is DD Form 2258?

DD Form 2258, Temporary Mail Disposition Instructions, is a form used by service members to inform the mailing clerks on the desired action with mail for the period of the soldier's absence. This form will be used to mark the mail receptacle of the temporarily absent service member. During this period the receptacle will be checked for mail or nonuse on a monthly basis.

The latest edition of the form - sometimes confused with DA Form 2258, Depreservation Guide for Vehicles and Equipment - was released by the Department of Defense (DoD) on January 1, 1982 . The most recent DD Form 2258 fillable version can be downloaded below or found on the Executive Services Directorate website.

Service members can also designate an agent that will receive their mail while they are absent using PS Form 3801. This form authorizes an appointed person or multiple people to receive mail for the person completing the form until they revoke this responsibility.

How to Fill Out DD Form 2258?

Additional guidelines and information can be found in the Army Regulation 600-8-3, Postal Operations, released in February 2016. DD Form 2258 instructions are as follows:

  1. The first two blocks of the form require the service member's full name and the receptacle number;
  2. The reason for absence is chosen from the options in the "Status" block by checking the appropriate box;
  3. The next block is for specifying the period of the leave. It cannot exceed 30 days unless the orders are attached to the form. Two boxes below allow the service member to choose whether they would like their mail to be forwarded to a temporary address or be kept on hold;
  4. The service member can choose specific types of mail to be forwarded to the temporary receptacle;
  5. The next box should contain the complete address where the chosen types of mail will be forwarded.
  6. Any special remarks or instructions can be provided on the next line. The instructions must contain the name of the sponsor and a duty phone for advance receptacle assignment;
  7. The receptacle holder should sign and date the form. The completed form should be turned in at a PSC window or deposited in a dropbox.
