Vehicle registration in Oklahoma starts from filling out Form 701-6. It consists of several parts, which include information, such as:

• Information about the filer and their vehicle . In the first part of the document, an applicant is supposed to designate their vehicle's body type, identification number, year and model, etc. In addition to this, filers must state their name, address, and driver's license number. If a vehicle is owned by an entity, then their Federal Employer's Identification Number must be stated in this field;
• Information about the lien . Here an applicant must enter the lienholder's name, their full address (including city, state, and ZIP), and date of the lien;
• Information about the fee for used tire recycling . This section is for reporting information about the tire size. The fee is assessed upon a vehicle's titling;
• Declaration of damage or theft . The declaration must be filled out for those vehicles which are 10 years old or newer. Here an applicant must designate if a vehicle was damaged by a collision or other occurrence, has it been recovered from theft, etc.;
• Flood damage disclosure . All applicants must fill out this section, regardless of how old their vehicle is. They must state if the vehicle suffered from water or flood damage. The section also provides an applicant with the criteria describing the damage and requires an applicant to answer if the vehicle meets the mentioned criteria;
• Vehicle inspection . The last section of the application is supposed to be filled in by the vehicle inspector. Here they designate the vehicle identification number and odometer reading. After performing an inspection of a vehicle they certify the designated data and sign the document. A vehicle inspector must also state their name, title or position, and their motor license agent number.
