HAZARDOUS WASTE TRANSPORTER REGISTRATION CHECKLIST
If you wish to apply for a new registration (e.g. if there was a change to your company status) or to renew
your current registration as a hazardous waste transporter, you are required to submit the following
documents:
Hazardous Waste Transporter Registration Application (DTSC Form 187)
Note: A “wet signature” original is required.
Proof of Insurance (ONE of the following is required)
o MCS-90 (Endorsement for Motor Carrier Policies of Insurance for Public Liability)
o MCS-82 (Motor Carrier Surety Bond for Public Liability)
o DTSC Form 8038 (Certificate of insurance for Public Liability Coverage)
o Letter of self-insurance (Government Agencies Only)
Supplemental Information of Insurance Coverage (if applicable)
o DTSC Form 1309 (if ALL vehicles are under 10,000 lbs and you do not transport oil in bulk)
Active EPA ID number (ONE of the following is required)
o Write existing EPA ID Number in box 8 on Transporter Registration Application (DTSC187)
o DTSC Form 1358 for non-RCRA waste (e.g., oil, asbestos)
o 8700-12 to obtain an EPA ID Number for RCRA waste
Consolidated Transporter Notification Form (if applicable)
o DTSC Form 1299
Transportation Regulatory Exemption (TRE) Application / Variance Form (if applicable)
o TRE Variance Form 1294
Once documents are complete, please mail the correspondence to the following address:
