PART 1 - INSTRUCTIONS FOR COMPLETING THE Y-10 FORM
CITY OR TOWN: Enter the name of the city or town or regional school district submitting the report.
SCHOOL YEAR: Enter the last two digits of the years covered by this report (for example, 99/00).
DATE OF REPORT: Enter the date the report was prepared.
NAME, ADDRESS, AND PHONE NUMBER OF TRANSPORTATION COORDINATOR: Enter the data requested pertaining
to the person coordinating public transportation for the city/town or regional school district.
CONTRACTOR DATA: Enter the name, address, and phone number of each contractor, to include individuals, companies or
community-owned providers of pupil transportation for your school system. Each provider and the provider's vehicles must be
listed separately.
VEHICLE DATA: Enter the total number of vehicles (in parentheses) by type (Type I; Type II; non-school buses), and the
Connecticut vehicle registration plate numbers for each vehicle in the columns provided.
PART 2 - TERM DEFINITIONS
CONTRACTORS: Any person, company, or government agency that provides vehicles for the transportation of school
children, to include non-mobility impaired and mobility impaired students, on a regular basis.
SCHOOL BUS: Briefly stated, a vehicle which meets the following requirements is classified as a school bus; for a more
detailed description refer to Section 14-275 of the Connecticut General Statutes.
COLOR: Yellow (National School Bus Glossy Yellow).
IDENTIFICATION: Must have the words "SCHOOL BUS" in black letters on the front and rear of the vehicle.
TYPE I: A Type I school bus has a Gross Vehicle Weight (GVW) exceeding 10,000 lbs.
TYPE II: A Type II school bus has a Gross Vehicle Weight (GVW) of less than 10,000 lbs.
NON-SCHOOL BUSES: Any vehicle under contract to a city/town or regional district to transport children to and from school
in a vehicle other than a school bus.
MOBILITY IMPAIRED: A car, station wagon, or van under contract to provide transportation to mobility impaired students.
NON-MOBILITY IMPAIRED: Any vehicle with a capacity of 10 or more passengers under contract to carry mobility impaired
students to and from school to include field trips (normally registered as a service bus).
VEHICLES: Less than 10 passengers including the driver must be registered with Combination Plates.
