Interpreter and/or Translator Certification Form
This form must be submitted with your “Application to Provide Interpretation and/or Translation Services” application
(DL-200). Please read the following MVA information and complete the certification section at the bottom of this form.
If approved and placed on the Maryland Motor Vehicle Administration’s (MVA) listing as an approved interpreter and/or
translator you must agree to and follow the MVA guidelines listed below. Failure to comply will result in the removal of
your name from the MVA Approved Interpreter and/or Translator Listing.
READ AND INITIAL EACH STATEMENT BELOW. Any statement without acknowledgement of reading and
understanding, by placing your initials, is subject to denial.
• I understand that an MVA representative, at any time, may monitor, record, and/or investigate MVA transactions
that are being conducted. ______
• When interpretation and/or translation services are performed at any MVA location, I will present a photo
identification card (valid state driver’s license or valid state identification card) to the MVA branch office staff.
The MVA staff will confirm that I am on the MVA’s Approved Interpreter/Translator Provider Listing. ______
• When interpretation and/or translation services are performed, I will follow MVA guidelines and instructions and
will not coach or otherwise inappropriately assist in an unethical manner, the Driver’s License Applicant during
any MVA examination process. _______
• I will ONLY read the questions as written for the Driver’s License Knowledge Test._______
• I will ONLY interpret the exact instructions given by the Driver’s License Examiner to the applicant during a
Driver’s Skills and Road Test. _______
• I understand that allegations of suspected fraud or misconduct are subject to an internal review and/or
investigation and may result in my removal from the MVA’s Approved Interpreter/Translator Provider
Listing. ______
• I understand I cannot provide interpretation and/or translation services for family members, including but not
limited to a blood relation or a relative related by marriage. ______
• If approved and listed as an eligible provider of interpretation and/or translation services for customers of the
MVA, I understand that the MVA does not specifically endorse, recommend, promote, or certify any of the
individuals listed. Furthermore, I understand I am prohibited from using such statements to advertise or promote
my interpretation and/or translation services. _______
• I understand I am prohibited from soliciting on MVA property (Maryland Law Transportation Article 12-117). ______
• I understand the use of the MVA or State of Maryland Logo is strictly prohibited. Examples include, but are not
limited to: badges, business cards, translated documents, etc. _______
• I will NOT use any cellular and/or electronic device while providing interpreting services during the knowledge
and/or skills test. _______
• I will NOT schedule any interpreting services that may interfere with the operation of the MVA. _______
• I understand if I am providing language services to an applicant, I may be prohibited from providing other
services that could be considered a conflict of interest. _______
“My signature certifies that I request to be listed as an eligible provider of interpretation and/or translation
services for customers of the MVA. I certify that I understand and agree to the above conditions and
guidelines of the MVA.”
