ticket, supporting affidavit, and consent to
adjudication by a Judicial Hearing Officer (JHO)

I, ______________________________________________, swear or affirm that:
I received a parking ticket for illegally parking in a handicapped parking space on
 date ticket
_________________________  .  On that day, I or 
was issued 
insert the name of the holder of the handicapped parking
__________________________________ had a valid handicapped parking permit, and was
permit 
using the car that was ticketed as a driver or passenger. 
The  handicapped parking permit was issued by 
insert name of town or agency which issued the permit

Based upon the foregoing, I now move to have said ticket dismissed on “proof.” 
In support of my motion to dismiss the ticket, I also submit the attached:
1.
photocopy of the permit, 
2.
letter from the town or other issuing agency stating that the handicapped permit was valid
on the date the ticket was issued,
3.
photocopy of my driver’s license or some other photo identification if you do not have
license, and
4.
a self‐addressed stamped envelope.

license or other photo identification of the permit holder .
I hereby consent to having this motion presented to a Judicial Hearing Officer (JHO) for
decision.  I understand that I have the right to have this case adjudicated before a District Court
Judge and at my own expense to have an Attorney represent me.
I understand that in deciding my motion to dismiss my handicapped parking ticket on
“proof” the JHO shall have the same powers as a District Court Judge and any action taken by the
JHO shall be deemed the action of the District Court and that I shall have the same right to appeal
as if my motion had been decided by a District Court Judge.
