Birth Record
The state registrar of vital statistics shall list the parents as decided above on the child’s birth
certificate and any other birth record.
Important! The court does not forward this order to the state registrar. A party must do this.
If the child was born in Washington State, a party must mail a certified copy of this Order, with the filing fee,
to the
state registrar of vital
statistics. Center for Health Statistics, Department of Health, P.O. Box 9709,
Olympia, WA 98507. (For more information call (360) 236-4300.)
If the child was not born in Washington, contact the appropriate agency in the state where the child was
born.
3.
Enforcement stayed until birth
This order cannot be enforced (is stayed) until birth of the child conceived through
gestational surrogacy or assisted reproduction.
