The IRS Form 433-F instructions are as follows:

  1. Identify yourself - write down your full name, address, county where you currently live, multiple telephone numbers, and the taxpayer identification number. If you are married, record the same information for your spouse. Indicate how many members of your household will be claimed on the income statement you file this year. In case you or your spouse have self-employment revenue or either of you is self-employed, list the details of the business you manage.

  2. Describe your accounts and lines of credit. You have to provide information about your bank accounts (different types - checking, savings, online, etc.), investments, bonds, stocks, and cryptocurrency. List the real property you own and specify what house or apartment you are using as your primary residence.

  3. Add other assets you own - from vehicles to business assets - and write down information about your credit cards. If you or your business is about to receive payments from your customers, specify the amounts you are owed and list the names of individuals and businesses whose names are on accounts receivable.

  4. Identify your employer and your spouse's employer as well - tell the IRS how often both of you get paid, how long you have been employed, and how much tax you pay on a state and local level. Confirm the amount of income your household receives apart from those wages including child support, social security benefits, and dividends.

  5. Provide a detailed breakdown of your monthly expenses - explain how much money you spend on food, housing, utility bills, transportation, and healthcare. There are additional fields that will let you clarify whether you are making insurance payments or taking care of student loans.

  6. Certify the form by signing and dating it. Attach as many extra sheets of paper as you need to share more information. You will also have to enclose documentation that further proves your claims such as pay stubs, profit and loss statements, and tax instruments that elaborate on your financial standing.

  3. Send the statement to the Internal Revenue Service Center, Austin, TX 73301-0010 if you reside in Florida, Louisiana, Mississippi, or Texas.
