Form W-147Q Instructions

Form W-147Q instructions are as follows:

  1. Enter the name of the primary tenant and their full address. If you have filed a public housing application with the New York City Housing Authority and you have a case number from that document, record the case number.
  2. State the name and address of the secondary tenant.
  3. Confirm you are the primary tenant of the property - describe it by writing down its address the way it appears in your lease. Certify the secondary tenant lives with you and shares these premises.
  4. Indicate the sum of money you are charging the secondary tenant each month for providing them with residence and necessary utilities.
  5. Enclose a copy of your lease or another document that proves you are a legal tenant of the apartment or house in question.
  6. Check the appropriate box to explain how the rent is paid - the secondary tenant might give the money directly to the landlord or pay the rent to you.
  7. If you share meals with the other tenant, answer "yes" by checking the box.
  8. Confirm all the statements in the form are true and correct to the best of your knowledge, sign the form, add your telephone number, and write down the actual date.

There is no need to obtain a signature of the secondary tenant - your personal statement is enough. Send this form to your landlord via email or traditional mail to notify them that you have decided to share a house or apartment with another person.
