What Is Form TC-123?

Form TC-123, Application For Utah Duplicate Title , is a form that must be filled out and submitted by the owner of a vehicle to apply for a duplicate certificate of title in the state of Utah. This is necessary to do if the original certificate has been damaged or lost. The Utah Division of Motor Vehicles delivers the duplicated title to an applicant a week after receiving such a request.

The owner of a car can use a Utah Duplicate Title Form if they sell it to a Utah resident. In this case, the owner must fill out this application and transfer it to the purchaser.

How to Get a Duplicate Title in Utah?

The TC-123 Form has to be filled in as follows:

  1. Part 1, Application for Duplicate Utah Title.

    • Section 1. The applicant has to enter information about the primary owner of the car, such as their full name and street address. Indicate their mailing address if it differs from the one you entered. Specify the co-owner's name and provide their address on the back of the application. If a car belongs to an organization, the filer should enter the business name of this entity;
    • Section 2. Provide information about a vehicle, such as a Vehicle Identification Number (VIN) or a Hull Identification Number (HIN). Indicate the year of manufacture, make of the car, and its model;
    • Section 3. Each owner should sign the application and indicate the date of completion. By signing this Section, they certify that the original certificate is lost or damaged and that they have requested a duplicate;
    • Section 4. Provide information about the lienholder. Enter their name and mailing address. A business name should be entered if the lienholder is an organization. This section must contain the signature of the lienholder and information about a title.
  2. Part 2, Assignment and Release of Interest.

    • Section 1. The odometer reading of a car has to be entered. The filer must select the appropriate checkbox regarding how it reflects its actual mileage. Indicate the sale details, such as the date of the transaction and price. Each owner should sign the application, thus confirming the transfer of ownership to the purchaser and the accuracy of the information provided;
    • Section 2. Indicate the details about the new proprietor of a car, such as their full name and street address. Specify the personal information about the new lienholder. Enter the business names of the new owner and lienholder if they are not individuals but entities. The new proprietor of this vehicle has to sign the form and enter its indication date.

If the purpose of filling out this form is to transfer the rights to a car to a new proprietor, the seller should sign Part 1 and Part 2 of this form.
