Form TC-123, Application For Utah Duplicate Title, is a form that must be filled out and submitted by the owner of a vehicle to apply for a duplicate certificate of title in the state of Utah. This is necessary to do if the original certificate has been damaged or lost. The Utah Division of Motor Vehicles delivers the duplicated title to an applicant a week after receiving such a request.
The owner of a car can use a Utah Duplicate Title Form if they sell it to a Utah resident. In this case, the owner must fill out this application and transfer it to the purchaser.
