Use one chart for each child. Figure the child’s age on the day you are checking the record. The child should have the doses recommended up to the child’s current age.
Each box with a number represents the dose number due at that age.
Put a checkmark in each box when the child has received that dose or write the date the child received each dose in the appropriate box.
Notes:
This chart shows the ages when the doses are due.
By age 2years children should have completed 3 doses of HepB vaccine, 4 doses of DTaP vaccine, 3 or 4 doses of Hib vaccine, 3 doses of polio vaccine, 4 doses of PCV13
vaccine, 1 dose of MMR and 1 dose of varicella. HepA vaccine may not be completed until later. Then, no more doses are required until kindergarten.

Children may receive 3 or 4 doses of Hib vaccine depending on the brand of vaccine used. ActHIB
is a 4 dose series. PedvaxHib
is a 3 dose series
Some children may receive 4 doses of HepB vaccine if a combination vaccine containing HepB is given. This is OK; record the extra dose under 4 mos. of age.
