Medical Power of Attorney Form - Colorado

A Colorado Medical Power of Attorney Form provides permission (by the principal) for another party (known as the agent) to make important medical decisions on behalf of the principal. The principal may opt to do this in case they feel there is a risk that they will become incapacitated or develop a condition which will prevent them from thinking and acting rationally. For this reason, the principal will often select an immediate family member or a close friend to act as their agent. When selecting an agent, it is important to be aware that the agent needs to be someone reliable and someone that can be contacted promptly should any form of emergency occur.

In Colorado, a Colorado Medical Power of Attorney only needs to be signed by the principal. There is no legal requirement for other parties to be included although this is highly recommended - either a notary public or two witnesses.
