DA Form 3020-R, the Magazine Data Card (MDC) is a form used for recording all information about the munitions issued, received, stored or held in stock within the United States Army.

The MDC - often confused with the DD Form 3020, Breaching Reconnaissance Record - is a non-regulated document and can be printed on demand. The letter R following the number of the form stands for "reproducible".
