DD Form 2958, Service Member Career Readiness Standards/Individual Transition Plan Checklist is an ITP checklist called the Capstone. The Capstone is filled out by the service member during the Transition Assistance Program (TAP) briefing to assure that they were actually provided the training.
This form - often incorrectly referred to as DA form 2958 - was released by the Department of Defense (DoD) in August 2013. A DD Form 2958 fillable copy is available for download below. Service members will also receive a printed version of the checklist during their transition program training.
Before quitting the military, all service members have to undergo Transition Assistance. The DD Form 2958 is submitted to the command along with other paperwork and is eventually used to prove to Congress that the retiring service member obtained the required training.
The form is used to make sure retirees are acquainted with all the benefits that they have the right to have access to, including VA or Job Center benefits. It also serves to prove that they have been counseled on all of the issues of financial, personal or family needs.
