DA Form 3645 Organizational Clothing and Individual Equipment Record

What Is DA Form 3645?

DA Form 3645, Organizational Clothing and Individual Equipment Record , is a form used to record transactions of Organizational Clothing and Individual Equipment (OCIE) between service members and the Property Book Officer (PBO). The form is also used to assign responsibility for the OCIE. The transactions include both issues and turn-ins. In the U.S. Army National Guard, the DA 3645 is sometimes used for temporary issues of gears to support annual training.

The latest version of the form - sometimes incorrectly referred to as DD Form 3645 - was released by the U.S. Department of the Army (DA) on October 1, 1991 , with the December 1983-issued forms used until exhausted. DA Form 3645-1, Additional Organizational and Individual Equipment Record, is commonly used for a similar purpose as the DA 3645. DA Form 3645-1 typically records additional items stocked and handed out by the OCIE issue point.

How to Fill Out DA Form 3645?

The form is regulated and controlled by the Department of the Army Pamphlet 710-2-1, Using Unit Supply System (Manual Procedures). DA Form 3645 instructions are as follows:

  1. This form is authorized for overprinting and should be filled either digitally or in ink ("MOS" - pencil, "Auth Alw." - ink or pencil).
  2. Providing authorized allowance is required for each item in the corresponding column.
  3. The description of the issued items should be provided in the "Closing and Equipment" block.
  4. The form requires indicating the quantities of every item possessed by the individual.
  5. The reverse side of the form must contain the individual's signature and the date of signing.

DA Form 3645 is prepared in three copies. The first copy is for the military personnel records, and the second is sent to the Soldier's unit for filling. These two copies are kept by the OCIE issue point. The third one is retained by the individual. The form remains in effect until re-issued by the OCIE issue point.

The list may also include other gear issued by the CIF. All OCIE prepared for turn-in must be clean and in serviceable condition. CIF does not accept turn-ins that are not properly cleaned.
