DA Form 3645, Organizational Clothing And Individual Equipment Record is a form used to record transactions of organizational clothing and individual equipment (OCIE) between service members and the PBO. The form is also used to assign the responsibility for the OCIE.
The transactions include both issues and turn-ins. In the U.S. Army National Guard, the DA 3645 is sometimes used for temporary issues of gears to support annual training.
