What Is GSA Form 1649?

GSA Form 1649, Notification of Federal Supply Schedule Improvement, is a legal document completed to authorize an alteration in a Federal Supply Schedule - a long-term government contract with a commercial enterprise that sells goods and services to the federal agency.

Schedules give federal agencies a simpler way to obtain commonly used supplies and services from a variety of contractors. However, sometimes a schedule must be modified to adjust the price of the contract, reconsider the volume of the purchases, length of the contract period, or delivery practices. Use GSA Form 1649 to notify the General Services Administration (GSA) of any change that relates to the acquisition of commercial goods and services. The contracting officer must coordinate and obtain approval of the GSA's Federal Supply Service (FSS) before implementing any modifications.

GSA Form 1649 Instructions

Follow these steps to complete the GSA 1649 Form:

  1. State the schedule number and title;
  2. Add the Federal Supply Group or Federal Supply Class the schedule belongs to;
  3. Enter the proposed effective date of the schedule;
  4. Identify the contracting officer responsible for establishing and changing the schedule, adding and deleting special item numbers and national stock numbers, and revising their description. Add the officer's correspondence symbol to indicate their place of assignment;
  5. Provide the updated information about the schedule - special item numbers (SINs) or national stock numbers (NSNs), number of contracts, dates, the total amount of items, and Federal Supply Class of each item;
  6. List the SINs and product lines and enter their actual or estimated SIN value;
  7. Describe the impact of the improvement on the current schedule;
  8. Indicate the dollar value of the schedule, adjusted value, and remaining value;
  9. Provide a full description of the improvement and its justification;
  10. Use the second page of the form if you need to write down additional information;
  11. Sign and date the form.
