DA Form 3739 Application for Compassionate Actions

What Is DA Form 3739?

DA Form 3739, Application for Compassionate Actions is a form used to request a compassionate action if the soldier has extreme family problems. This may be a reassignment, detention, a deferment or a permissive attachment based on the specific circumstances.

The latest release of the form - often incorrectly referred to as the DD Form 3739 - was introduced by the Department of the Army (DA) in August 2018. An up-to-date DA Form 3739 fillable version is available for digital filing and download below or can be found through the Army Publishing Directorate website.

There are two types of compassionate requests depending on the essence and possible circumstance of the personal problem the soldier is facing. The two types of requests are requests due to temporary situations, expected to be resolved within a year, and requests due to permanent situations.

Health-related compassionate actions are reviewed by the Office of the Surgeon General with a special recommendation forwarded to the DA Human Resources Command (HRC). The HRC will review and respond all requests via an Enlisted Distribution and Assignment System (EDAS) vice electronic message within 7 to 21 days after receiving the application. Service members should check with their S-1 for the status of their request.
