What Is PS Form 1000?

PS Form 1000, Domestic Claim or Registered Mail Inquiry - also known as the USPS claim for loss or damage - is a form used for filing a claim for recovery of property or funds if a package was lost, arrived with missing contents or damaged.

The form - often referred to as the USPS Form 1000 - was last updated by the United States Postal Service (USPS) on March 1, 2005 . A printable version of the form is available below for reference. As of today, no PS Form 1000 fillable copies can be found online or on the USPS website.

USPS Domestic Claim Form

There are two types of claims: domestic and international. Domestic claims are filed for packages sent within the U.S. If the parcel arrived without some contents, was damaged or lost, the claim should be filed as soon as possible. Domestic claims can be filed online using the USPS website. If the parcel arrived damaged or with items missing, the claim should be filed within 60 days from the arrival. The filing window for claims for lost mail or packages varies depending on the mailing service.

Filing a claim is possible only if you have an account on the USPS website. You should provide the tracking number and shipping date, information about the package and a reason for your claim. Indicate, whether you are the sender or receiver.

Provide a list of your lost or damaged items. You can add up to 10 items. Select the type for every item and briefly describe them. Provide the date the items were purchased and the amount of a sufficient compensation payment you claim.

Provide proof of value. Proving files can be uploaded from your computer in PDF or JPEG format. The options of proofs of value are as follows: a sales receipt (if applicable), invoice or bill of sale, or statement of costs for the reconstruction of Express Mail service documents. After describing the items, the brief name is given to the claim for easier identification.

Review the claim carefully before submitting it. After submitting the confirmation will be sent to your email address. The details and status of the claim can be easily checked in the Claims History page with the brief name given.

If the claim is denied or the payment seems insufficient, the decision may be appealed. The additional information to be considered is entered to the claim on the Claims History page.

The PS Form 1000 is a domestic claim form. If filing a claim online is unavailable, then the claimant can call 1-800-332-0317 to request a form. During the call, the automatic voice response suggests selecting an option. Select Option 4 to order a blank of the PS Form 1000 and then select Option 4 again. The form should be mailed to the address provided on it with supporting documentation, proving value, and insurance.

International claims are more complex. Only US senders can initiate an inquiry process. This can be done online, but only within a filing window, which depends on the mailing service. Some mail services are available for refunds and ineligible for claims. Processing international claims take more time since it requires coordination with foreign mail administration. To file a claim, the sender needs a 13-digit tracking number, proof of damage and evidence of value.
