What Is SC Form 400?

Form 400, Title Application , is a form that allows the new owner or lessee of a vehicle to apply for a South Carolina vehicle title - a legal document that establishes the individual as the legal operator of the vehicle and provides proof of ownership of the vehicle. You are required to apply for the title within 45 days of the date of purchase or the date of the vehicle operation in South Carolina.

SC Form 400 Instructions

Include the following information in the SC Application for Title:

  1. Select all the boxes that apply to your situation, whether you need a new title or plate, record a lien or obtain a duplicate title.
  2. Describe the vehicle - add its identification number, make, model, year, body style, fuel type, empty weight, gross vehicle weight. If you are applying for a moped, write down its engine cubic centimeters or wattage.
  3. Indicate the current odometer reading or state the odometer discrepancy.
  4. Provide the new owner or lessee information - full name, driver's license number, social security number (SSN), or federal employer identification number (FEIN), date of birth, residence, mailing, and temporary addresses, address where the vehicle is housed, and the telephone number.
  5. If the vehicle is leased, name the leasing company and add its contact information.
  6. If there is an outstanding lien on the vehicle, enter the lienholder's name, address, and the date of lien.
  7. If your vehicle is 26,000 lbs. or greater or it is a bus common carrier, you are required to disclose your SSN or FEIN.
  8. If you are entitled to a sales tax exemption, indicate the transferor and transferee of the vehicle. Check the appropriate boxes if you are active-duty military personnel or the vehicle was a bonafide gift.
  9. State the date the vehicle was purchased and first operated in the state. If it is an energy-efficient manufactured or mobile home, check the appropriate box. Indicate if the vehicle is new or used and record its prior tile number and title state.
  10. Include the seller information - the name of the seller or dealer, dealer's registration number, and address. Add the price of the sale and sales tax number.
  11. Name the insurance company that issued a liability insurance policy for the vehicle.
  12. If you wish to donate money to the Donate Life South Carolina, record the amount of donation.
  13. Certify you are the owner of the vehicle, sign and date the form.

There are two ways to apply for a South Carolina vehicle title:

  • Bring SC DMV Form 400 and other related documents to the SCDMV branch;
