What Is a TRiPS Offline Assessment Form?

A TRiPS Offline Assessment Form is a formal document prepared by a soldier before they travel, often in their own vehicle, with the purpose to inspect the vehicle and implement risk management controls if necessary. With the help of this tool, provided to army personnel by the Travel Risk Planning System (TRiPS), you will be able to identify and assess the risks that may occur even during a well-planned trip and calculate the most reasonable route to the destination of your choice.

TRiPSOffline Assessment Instructions

Follow these steps to complete the TRiPS Offline Form:

  1. Indicate the destination. Write down your driver's license number if you will be using your own vehicle to reach the destination.
  2. If the trip takes considerable time, you must plan rest stops and breaks and record them in the form.
  3. Check the weather forecast and state the anticipated weather conditions.
  4. Enter the distance of travel and the mode of travel if you will take a taxi, train, or plane.
  5. Record the point of departure and its date and time. Calculate the time of arrival or write down the time indicated in your train or plane ticket.
  6. Name the hotel you will be staying in, its location, and the date of check-in. Provide the same information for your return trip.
  7. State your name, rank, sign the form, and add the actual date.
  8. Bring the form to the vehicle inspector and let them check your car - they will complete the vehicle inspection checklist if you make the trip with your own car. It is required to list the main vehicle characteristics - year, make, and model. Every item listed in the table must be checked: look for deficiencies indicated in the worksheet and examine the car for issues its operator has encountered before. For each item, the inspector needs to check or circle "pass" or "fail" in the column on the right. After the inspection, they will add their name, state the date of inspection and follow-up inspection if necessary, and sign the form - you will add your name and signature as well.

Once the document is filled out, bring it to your supervisor. If you have doubts about certain sections and lines of the form, consult with the supervisor, and complete the form together. The supervisor, in their turn, will put their initials on the first page of the document once you have discussed risks and hazards and decided on the appropriate action to confirm their familiarization with the worksheet. They will answer "yes" to certify the inspection of the vehicle and your insurance coverage as well as the fact that you have a valid driver's license, write down their name and rank, sign and date the worksheet.
