Form DOR-108, Application for Missouri Title and License , is a formal document prepared by Missouri residents to obtain a motor vehicle title and license. How to Fill Out an Application for a Missouri Title and License?

Here is how you fill out the DOR-108 Form:

  1. Indicate the type of request - you need to transfer or renew plates, get new plates, or get a new title.
  2. If the vehicle owner is the same and you want to get a Missouri title and notify the authorities about the vehicle lien, put a tick in the appropriate box.
  3. Choose the type of title you want by checking the box.
  4. Provide the details of the vehicle owner - full name, address, telephone number, email. Write down your driver's license number or employer identification number.
  5. Describe the vehicle - you have to list its year, make, color, identification number, and mileage. If the car was bought, rebated, or traded-in, indicate the price.
  6. If you notify the authorities about the outstanding lien, provide the lienholder's name and contact information and state whether this is the first or second lien. You may also ask to send the title to the lienholder's address by checking the appropriate box if the lien will not be subject to future advances.
  7. Calculate the fees - it is recommended to do it in the presence of the license office representative. You may be asked to pay the license fee, reservation fee, and penalties in case you did not renew the plates or get a title on time. If you wish, you can donate to the Missouri funds.
  8. Record the details of the trade-in transfer by once again describing the main characteristics of the motor vehicle.
  9. Confirm you are responsible for the vehicle, certify all the statements in the form are true and correct, sign and date the application.
  10. If the vehicle was acquired from a dealer, they must state their number, name, contact details, and sign the form.
  11. If you are applying for a duplicate title, provide the reason for it, and notarize the application.
