Form SSA-24 Instructions

Provide the following information in the Survivors Benefits Application:

  1. Write down the full name of the veteran and their date of death. Indicate the veteran's social security number, date, and place of birth, name of the father, maiden name of the mother. If the veteran worked in the railroad industry after 1936, answer "yes" by checking the appropriate box.
  2. State the date the veteran entered active military service and separated from it. Enter the military service number and veteran's grade, rank, rating, organization, and branch of service.
  3. Explain your relationship to the veteran whether you are a spouse, divorced spouse, child, or parent. Record your date of birth. Enter the Veterans Affairs (VA) file number of the veteran.
  4. Name the surviving children or dependent grandchildren of the veteran who were unmarried and under 18, 18 to 19, and attending secondary school, or disabled at any time since the death of the veteran.
  5. Confirm the information in the application is true and complete to the best of your knowledge, enter the date of signing, print your name, and sign the document. Add your mailing address and telephone number to receive documentation from the SSA and VA.

  2. Birth certificate or marriage certificate;
  3. Divorce decree;
  4. Proof of citizenship or immigration status;
