Connecticut Certificate of Title Instructions

Completing the application is a straightforward process. The document consists of three sections:

  1. Applicant/vehicle and title information. Here an applicant is supposed to enter personal information and information about their vehicle.
  2. Lien status for a lien over ten years old. In this section, an applicant must designate if the lien has been satisfied.
  3. Applicant's signature and customer contact information. The last part of the form contains a place for a filer's signature and personal information.

The application makes the process of applying for a replacement certificate easier, however, there are a few intimidating moments that need to be underlined:

  • For a replacement title a filer must fill in the first and the third section;
  • If an applicant had a lien on the vehicle, they must enclose a letter from the lienholder stating the lien has been satisfied;
  • If the lien is over ten years old, a filer must complete the second section of the form indicating that the lien has been satisfied;
  • If a lien is still outstanding, the lienholder shall apply for a replacement certificate of title with a power of attorney;
  • If an applicant wants to file Form H-6B not only for a replacement title but also for an ownership transfer, they must submit Form Q1, Supplemental Assignment of Ownership, with it;
  • If a filer needs proof of ownership documents, they must file Form H-115, Motor Vehicle Ownership Affidavit.

After completing the document, a filer is supposed to mail it to the Connecticut DMV: the State of Connecticut, Department of Motor Vehicles, Room 305, 60 State Street, Wethersfield, CT 06161. A check for $25 made out to DMV must be attached to the document.
