What Is a Parental Consent Form?

A Parental Consent Form is a form filled in by legal guardians of a child which grants children permission to do certain things. There are several types of consent forms and they slightly differ depending on what they are required for. In most cases, consent will be required to allow a child to travel (Traveling Parental Consent Form) or permit the child to take part in certain activities. A consent form may also allow a third party (that is not a legal guardian) to decide on medical treatment should this be required in an emergency situation.

Most of the time parents will come across these forms in schools as they require them the most. In cases when a child under the legal age of 18 wants to get married, they can do so by asking a parent to sign a Parental Consent Form.

How to Make a Parental Consent Form?

Although there are various types of Parental Consent Forms, depending on what is being requested, they all follow a similar structure which should be included to produce a simple Parental Consent Form:

  1. The time period for which the form is effective. Usually, these are specified with a start and end date.
  2. Specific details regarding the duration of the trip or activity and information on where the child will stay during the whole period of the trip.
  3. Details about the person who is granted permission and will effectively be liable for the child. This includes their full name and contact details.
  4. Details about the parents including their full name and contact details.
  5. Details about the individual for who the permission is for (details of the child). This includes their name and date of birth.
  6. Specific details regarding what the permission is for. If it is a Traveling Parental Consent Form, it must include specific details on the final destination and any places where the child will be stopping and staying. For Medical Parental Consent Forms, maximal information should be included regarding any known medical conditions, any allergies a child may have, and any medication that a child needs to take.
  7. Any other emergency contact details. Should an emergency situation occur, the person liable for the child must have access to contact details of trusted people that they can get in touch with to inform them of any issues. The more people specified in the form, the better. Usually, this includes parents, grandparents, and other close friends or relatives.
  8. The legal representative(s) must sign and date the form and submit it where required.

All information in the consent forms must have answers to any questions that may be asked by a healthcare professional or legal representative to ensure a pleasant and problem-free trip for your child.
