DA Form 7566 Instructions

The old DA Form 7566 used to consist of thirteen fields on two pages of the form. Filing instructions were as follows:

1. Boxes 1 through 4 require general information about the event. Boxes 1 and 2 are used for describing the type and time of the event while Boxes 3 and 4 are used for listing the date of entry and the name, rank, and position of the person responsible for completing the form.
2. Boxes 5 through 12 - continued on the second page of the DA 7566 - provide the space for naming the subtasks of the event, followed by the risk assessment itself.
3. Box 6 is used to describe the hazards faced by the event. Boxes 7 through 11 overview the level of risk, controls, and supervision plans that were to be implemented. The person preparing the form had to indicate the levels of risk associated with the task as low, moderate, high, or extreme.
4. Box 12 is for determining if the controls and supervision were adequate and effective in suppressing the hazards.
5. Box 13 provides space for evaluating the overall risk level of the supervised tasks after the controls had been put in place. After the risks were assessed, the risk decision authority would sign off on DA Form 7566 before handing it off for recording and safekeeping.
