What Is SF-15 Form?

Form SF-15, Application for 10-point Veteran Preference , is a legal document needed to claim a 10-point veteran preference. Then, the U.S. Office of Personnel Management (OPM) determines if the applicant meets the requirements to receive this preference. This is a document for veterans (military retirees and individuals who have been honorably or generally discharged) and relatives of these veterans only.

What Is SF-15 Form Used For?

Veterans may have an edge in getting a civil service job using the preference points and veteran's recruitment appointment rule.

This preference applies to hiring from civil service examinations. First of all, to claim the preference, you must be a veteran who has served and was discharged or released from active duty in the armed forces under honorable conditions. Ten points are added to the passing examination score of any eligible individual:

  • You have served and have a present service-connected disability;
  • You have served and receive compensation, pension from the military or the Department of Veterans Affairs (VA), or disability retirement benefits;
  • You have received a Purple Heart;
  • You are an unmarried spouse of a deceased eligible veteran;
  • You are a spouse of an eligible veteran unable to work because of a service-connected disability;
  • You are a parent of an eligible veteran who is permanently disabled or died in service.

The SF-15 Form is used to apply for this preference along with the supporting documentation.

How to Fill Out SF-15 Form?

Form SF-15 instructions are as follows:

  1. Write down the full name and home address of the person applying for the preference.
  2. Provide the veteran information - full name, the branch of service, and period of service. If applicable, add the date of separation or release from the active duty.
  3. Indicate the type of 10-point veteran preference you want to claim. You have to answer the questions associated with each claim - whether you are currently married to the veteran, have you ever remarried, is your spouse totally and permanently disabled, etc. Depending on the basis of your claim, you will be asked to attach certain documents listed on the second page of the form.
  4. State the veteran's present occupation or military occupation at the time of separation. Specify if the veteran has been employed by the federal civil service identifying the position, agency, and dates of employment. If the veteran has resigned, separated, or been disqualified from a position because of a service-connected disability, submit the related documents. If the veteran receives a civil service retirement pension, answer yes in the appropriate box.

You may attach the following documents to the SF-15 Application for 10-point Veteran Preference:

  • Form DD-214, Certificate of Release or Discharge from Active Duty;

  • Separation and retirement orders;

  • Documents that establish transfers to the reserve corps;

  • Official statements from the military personnel records center;

  • Official statements from the VA certifying the service-connected disability or a disability pension;

  • Official certificate showing the award of a Purple Heart;

  • Documentation of a veteran's death - certified copy of the death certificate, and DD Form 1300, Report of Casualty;

  • Statements from the physicians showing the percentage of the disability;

  • Documents that prove the veteran's inability to work due to the service-connected disability;

  • Court decree of the annulment of remarriage.

Where Do I Send My SF-15 Form?

Certain disabled veterans, veterans who have served on active duty, and their closest relatives are entitled to preference over non-veterans in hiring from competitive lists. Note that the entitlement to veterans' preference does not guarantee you a job but SF-15 can give you an advantage in the hiring process.
