How to Fill Out a Colorado Motor Vehicle Power Of Attorney?

Form DR 2175 can be vaguely divided into several parts, namely:

1. The Parties. In the first part of the document, the grantor must state their name and the name of the agent. Each section of the form is accompanied by short notes that are supposed to help fill it out. For example, in the note for this part is stated that both names must be listed as they appear on the identification documents.
2. The Subject. Here, the grantor is supposed to designate what kind of rights they give to their appointed agent. They are offered several options from which they can choose one or more. By picking a particular option, the grantor can delegate their agent the right to apply for a new registration for their motor vehicle which they have stated above. In addition to this, the grantor is required to indicate the date when this power of attorney will be terminated and can no longer be in use.
3. Motor Vehicle Information. The grantor must use this section of the form to provide information about their motor vehicle.
4. Signature and Identification. Before signing this document, the grantor must acknowledge all of the statements the document contains. By singing it, they certify that the information presented in this power of attorney is true and valid. Additionally, the grantor must identify themselves with their Colorado Driver's License or Colorado ID.
5. The Notary Public. This part of the form must be completed by a Notary Public, who must validate it. To do so, they must sign the power of attorney, seal it, and fill in the gaps.
