What Is DA Form 2408-12?

DA Form 2408-12, Army Aviator's Flight Record , is a form used for recording flight operations and aircraft maintenance information. The data gathered in the form may later be provided to the Centralized Aviation Flight Record System (CAFRS) by the Logistics Information Systems (LIS).

The DA 2408-12 is a part of a series of 40 forms along with the more widely-known DA Form 2408, Equipment Log Assembly (Records).

How to Fill Out DA Form 2408-12?

DA Form 2408-12 is used with aircraft, simulators, and Unmanned Aircraft System (UAS) training devices to record the following information:

  1. Aircrew personnel data, duty symbols, aircraft type, time flying, and type of flight completed by the crew and pilot.
  2. HIT check deviations (if applicable).
  3. Operating hours, number of starts, and servicing data for the designated auxiliary power unit.
  4. Landings, landing gear cycles, and servicing data for the aircraft.
  5. Rounds fired.
  6. Internal and external loads.
  7. UAS rocket assist and pneumatic assist take-offs.
